ACSI now offers a check by phone payment process by which we can automatically debit your checking account.

Simply complete the form and either fax it to us, or scan the completed form and email it to

Is there a fee charged to pay this way?

No, this is a free service to you. However, if your payment is returned for any reason by your banking institution, a $25 processing fee will be added to your ACSI account.

Why do I have to submit a form?

Due to requirements set by ACSI’s banking institution, we must have proper documentation to support each transaction.

Can anyone complete this form?

The individual(s) authorized on your bank account should complete and sign this form, just as if they were signing a check!

What’s the time frame for this payment to be processed?

Forms received Monday through Friday before 3:00 PM MST will be processed that same day; you will see it debit your account the following business day. Forms received after 3:00 PM MST will be processed the following business day and you will see it debit your account two business days after submission.
