Annual reporting has been updated with the exciting announcement of ACSI's revised accreditation protocol, REACH (Reaching for Excellence through Accreditation and Continuous improvement for Higher achievement). Purpose & Timeline
During your accreditation term you will be required to submit an annual report as prompted by your regional office. Each annual report leads your program through the progressive improvements that will strengthen and reinforce the quality of your program.
With the launch of the new REACH accreditation protocol, a full accreditation term length is five years. A school/program granted the full accreditation term, will submit four annual reports before their accreditation renewal.
We encourage you to continue your investment by committing sufficient time to address the goals you outlined in your continuous school improvement plan and the recommendations offered by your visiting team.

Submission Instructions & Forms
1. Complete all forms applicable to your school/program. Please see chart below to review the list of forms that apply to you.

Note: If you need to calculate the percentage of staff certified in your school, use the certification auto tabulation form to help you. Please use this in-house only and not for your report sent to the Regional Office.
2. Attach supporting data (if applicable, not required):

Professional Qualifications: Deficiency Forms
If your program is accredited EE–12, you will need to complete the information detailed for both K–12 schools and EE programs. This information is only necessary if you have personnel who do not meet the qualifications detailed in the indicators for accreditation standard 4, personnel.
Information for K–12 Accredited Schools
If you have staff who have received an approved exception to the staff qualifications from your regional accreditation commission, complete the following forms and save them to your computer before inserting information.

Faculty Deficiency Action Plan
The Faculty Deficiency Action Plan must be completed by each faculty member who has received an exception from the regional accreditation commission.
If you have new faculty and staff that do not meet accreditation staff qualifications, you will need to submit a formal petition to your regional office. ACSI Policy states that exceptions are only granted during the candidacy period and accredited programs will hire only degreed and appropriately prepared staff. The regional commission will provide guidance to your school for short-term, extenuating circumstances.

Early Education Programs
Note: If you are a stand alone early education program that has answered no to section 4B, question one on Appendix C, and you are currently accredited with the 2000 Prechool Accreditation Instrument, there are no additional forms to complete. Skip to section 5 Finances.
This information becomes applicable only when a program becomes accredited with the 2008 accreditation instrument.
During the candidacy visit for the 2008 accreditation instrument the ACSI representative will discuss staff qualifications with the director of the program or school administrator. If a staff member does not meet the qualifications as stated in indicators 4.5a, 4.6a, and 4.6b, the ACSI representative will refer the director to Appendix T. Appendix T provides guidance on how the entirety of an individual’s experience, education, and expertise will be considered when reviewing qualifications.
The ACSI representative will assist the director to interpret the education, experience, and expertise of a staff member and create a professional development plan that will move the staff member to meet the accreditation requirements within the time periods listed in Appendix T. If any team member does not have a combination of 100 points using the chart in Appendix T or does not meet the accreditation indicators as listed in 4.5a, 4.6a, and 4.6b, this plan must be completed and submitted for approval.
Progress on the Professional Development Commitment must be included in the annual report after accreditation is granted on the 2008 instrument.

The ACSI Certification Report, Appendix F, is a part of the annual reporting submission requirements. These forms do not apply to early education directors or faculty, as well as, international faculty. These forms need to be completed and submitted by all K–12/EE–12 schools (including international administrators) with their REACH annual report.

The Annual Statement of Financial Practices form, Appendix J, is an accepted practices questionnaire. This will help you complete Section 5 of the annual report form. Continuous School Improvement Form
This is a new form that is being implemented as part of the launch of the new accreditation protocol. This will assist you in completing Section 6 of the annual report form. This one form captures your responses to two questions: 6.B & 6.C.
Schools and programs evaluated on instruments published prior to 2008 will only use a portion of the form—this will be expected by the regional commissions. By launching the forms now, school and program leadership will become accustomed to the new delivery system.

This form assists you in completing Section 6 A on the annual report form.
The first question in Section 6 A reads:
Is the school in compliance with all accreditation commitments and standards? (If no, attach an explanation of the commitment(s) or standard(s) not in compliance.)
Once approved by the 2008 accreditation instrument, this form becomes required if a program or school moves out of compliance during the accreditation term concerning an item for which a formal exception has not been received.

The early education exempt form applies to all accredited early education programs, inlcuding programs attached to a larger school that have ACSI joint accreditation.

If your school/program is accredited by another orgranization in addition to ACSI, they may require an annual report. ACSI will not file a report with another organization on your behalf.
